I would see ASAP: I am fortunate to be in area with multiple cardiologists that i coordinate care with daily. It sounds by history alone that the pt had a probable ischemic cardiac event in the evening. With no pain in the am, i would be comfortable seing the pt, obtaining stat labs, ekg, starting meds, and getting an emergent cardiology appt at the hospital for a probable cath. Hope this helps.

ER: Definitely er, if he would insist i would still see him, but in office cardiac evaluation is limited and consequences of missing a heart attack, or not treating it in time can be grave, so hopefully my patient would have enough common sense and wouldn't risk his life and would trust his doctor judgment, because if you don't trust your doctor than you definitely need to find the one you will trust.

Judgement: It seems that something happened that you are not happy about. If you are concerned go to the emergency room. If the doctor is familiar with the case, he/she might have reason to give appt, or doctor might have not appreciated urgency of request. Most such requests are handled without physician knowledge, by non medical staff.
