About equal: Depends on whether you inhale the smoke. Pipe tobacco has more tar, but fewer people inhale it, while cigar tobacco has less tar, but is more commonly inhaled. Both are associated with high rates of cancer of the mouth, lips, tongue and throat, but slightly lower rates of heart attack than cigarette smoking, and way less emphysema and bronchitis in those who do not inhale the smoke.
