Likely: Wow its very rare to have both together. Yes ultimately will likely need surgery, but should discuss with endocrinologist first.

Not rare: It is not unusual to see high tpo antibodies (typically associated with hashimoto's) in people with graves disease. However, it does not change the treatment. If you have not gone into remission after 2 years of methimazole, it is unlikely that further treatment with this med will work. You will likely need definitive treatment with radioactive iodine or surgery.
