At times this is optional - as in a hernia that is not stuck in the wrong place, or a nose that is simply not a pleasing shape. In other circumstances it is an emergency, such as when there is a hole in a loop of intestine and stool is spilling into the abdomen. The urgency of "needing" surgery depends on the severity of the disease.
