Facial surgery: Broad question and difficult to answer in this format. Keep in mind plastic surgery is both reconstructive and cosmetic. If your plastic surgeon feels that your potential surgery is reconstructive in nature, then your insurance provider is more likely to cover the procedure. Additional tests or photographs may also be required.

Facial Plastic Surge: Obviously, this depends on your insurance. Cosmetic procedures are generally not covered. Procedures which improve the function of facial structures and certain reconstructive procedures may be covered. See a facial plastic surgeon for more information.

Depends on procedure: Cosmetic procedures generally are not covered by any insurance. If there is a functional limitation (breathing impairment from crooked nose, significant visual obstruction from drooping eyelids) then coverage may apply. Good start is to talk with a facial plastic surgeon and call your insurance carrier directly concerning the proposed procedure.

Functional/reconstr: It would be best for you to be examined by a specialist who can compose a letter of predetermination of benefits to ascertain your level of coverage by your third party payor. Typically companies will pay for reconstructive or functional surgery but not for elective cosmetic surgery.

Depends: Depends on the problem and your insurance company. Look at your benefits brochure. Rarely purely cosmetic procedures are covered. If your problem is due to injury or developmental problem, much bettee chance of coverage.
