LIPS: If you mean the urethral meatus or the opening for the urine it can inflame from topical irritation, allergies to soaps, lotions or detergents. Not likely related to a hernia.

Unclear : Not sure if you mean the skin is split open, or as in the case of a large inguinal hernia, the scrotum could swell and become larger than the penis, potentially causing the appearance you have noticed. Sounds like you need to see a doctor for a physical exam.
