Couple thots: You may be experiencing pain primarily within the temporomandibular joint (tmj syndrome), that is referred to the ear. A dental eval may help to confirm. A rare condition called "nervous intermedius neuralgia" typically causes intense pain lasting several minutes to hours, and can be confirmed if loss of taste on the same side. An ENT doc may help with this. Mastoid problems may be considered.
