Hard to say: I would start with a ear, nose and throat specialist. This could be anything from a a tumor near the ear canal (ocoustic neuroma) to chronic otitus (an inflammation inside the middle ear). You might need an MRI or ct to start the diagnostic process. Sometime dental issues get referred to the ear, also.
