Leave it alone: Normal boys are sensitive about having their foreskin pulled back. Most foreskins separate on their own as time goes go by. He likely has what is called "physiologic phimosis" with normal attachment of foreskin to head of penis plus a somewhat narrow opening. Discuss with your pediatrician if foreskin opening is only pinhole in size, or if foreskin balloons or is painful with urination.

Phimosis: Your little boy may have phimosis which affects 3% of boys. The tip of the foreskin is too tight to allow the head of the penis to come out. I recumbent taking him to a pediatric urologist as there are many treatment options- including surgery.

Be gentle: It is normal for an uncircumcised boy to have "phimosis" trapping the head of the penis within the foreskin. Aggressively retracting the foreskin to force the glans out or tearing at the adhesions can cause significant pain and lingering problems including recurrent adhesions. If he only complains when it is "washed" then leave it alone. If he complains at other times, then see your md.
