Joint and/or nerves: Severe pain triggered by arm elevation suggests a joint problem. Rotator cuff injury can do this. However, the brachial plexus is a collection of nerves under the clavicle that can cause severe shooting pains into the upper arm. Inflammation of the plexis can produce symptoms as you describe. A thorough medical evaluation would be appropriate.
