Various things: Pain by itself may be of concern, but if you have other symptoms (fever, chills, night sweating, redness, tenderness), then a testicular torsion, testicular infection, hydrocele, varicocele, etc, are all possibility, while cancer is the least likely. Generally, cancer is a painless mass. Still, you should probably have it checked out.
