Instability: Its hard to know if you are describing instability or a separation(ac sprain) if you are subluxing due to instability you might be able to rehab it or you might need a repair if you fall on your shoulder you get get an ac separation or sprain from the fall you need to get a more specific diagnosis.

Ortho Eval needed: A through upper extremity evaluation along with appropriate imaging by an orthopaedic surgeon specializing in shoulder injuries should be able to define whether or not your shoulder instability would be best managed with physical therapy or potential stabilizing surgery

No: Continuing to separate your shoulder can lead to arthritis very quickly. If that is going on, I would recommend a shoulder specialist in Orthopedic surgery to evaluate you.
