How can i tell the difference between arthritis or just back pain?

MRI: Lower back pain is caused by many factors (back arthritis, muscle spasms, ligament tears). To differentiate simple muscle spams from lower back arthritis ( disc disease, joint degeneration, ligament overgrowth, etc), you will need a lumbar spine mri.

I have tried NSAIDS . Was previously on 800 mg ibuprofen 3x a day. Caused stomach issues. Was put on tramadol. No difference in pain despite taking it 4x a day. Have been given cortisone shots . Helps with pain for approx 2 weeks, then back to pain and discomfort. Pain scale varies between 4 to 7 on daily basis depending on activity. I need to lose weight but knee pain prevents me from vigirous exercise Arthritis?
