A couple of things: Pain at the top of patella can result from a few things. First, the cartilage of the knee cap might be inflamed (chondromalacia). Second, you might have injured your quadriceps tendon or have tendonitis. Finally, bursitis can occur surrounding the knee cap, but it is less likely without trauma.
