CKD IV: It really depends on many factors. What caused his kidney disease, how rapidly it is progressing, what medications he is on. For some people with stable stage 4 kidney disease (ckd), they can stay at that level for years. For others, the disease is more aggressive and can advance quickly. Most people are in between. He needs to avoid meds that harm the kidney and follow with his doctor.
