Stroke: Perhaps you mean "aphasia", an impairment of language ability? Someone who's had a stroke, who has a brain tumor, or even CNS infection may show confusion and aphasia. There are several types of aphasia -- such as expressive, where the person understands what's said but cannot articulate; and receptive, where nothing is understood. Selective impairments of reading or writing can happen too.
