Multiple possible: Disorientation to time or place is one of the signs of delirium, an acute mental status change that can be reversible with treatment. Accompanying signs are confusion, disorganized thinking, changes in alertness, decreased short-term memory & recall, perceptual changes including hallucinations, and wandering attention. Causes include intoxication, infection, brain injury, metabolic, and more.
