It depends: You cannot predict the severity of a reaction based on how severe a prior one was. So, if there is even the potential for a serious reaction (e.g., allergies to peanuts or shellfish), you should have an epipen (epinephrine). However, epipen (epinephrine)s should only be used if you have a reaction that causes swelling in the throat leading to difficulty breathing, and/or a drop in blood pressure.
