Consider sarcoidosis: Sacoidosis can cause diffuse micronodular changes in lung tissue and in the less common cases of cardiac infiltration a dilated nonischemic cardiomyopathy (may also cause a restrictive cm). Other possibilities include viral or other chronic inflammatory conditions which affect the lungs and heart simultaneously.
