YES, without care: If you do not seek care for your periodontal disease, the disease will continue to progress and you will lose more bone around the teeth, causing them to become loose and eventually you can lose your teeth. You can maintain the progression with frequent hygiene visits to disrupt the damaging biofilm/bacteria that cause it to progress. Your home care is paramount to success as well.

Periodontal disease : Untreated "moderate" periodontal disease advances over time, with additional bone loss around the teeth, the teeth loosen and are eventually lost.

No.: Moderate periodontitis will usually not result in tooth loss - assuming it stays moderate. It should be treated definitively. If it is not, it can progress to severe disease with increased bone loss which will lead to tooth loss. Having regular cleanings or even scaling is not the appropriate treatment for disease at the moderate level and will only appear to control the disease.

Not likely: Your teeth will not fall out unless you do not get treatment and the disease continues to progress. You need to see a dentist who will help you control the infection and then treat the symptoms (scaling & root planing). Procedures today can eliminate the infection and get tissue regeneration so the pockets get shallower or are eliminated.
