Could be a disc : Problem if the headache is in the back of the head coming up from the neck with pain over the shoulder. Or shoulder blade into the arm and about the front of the shoulder about your breast. Cardiac issues can also present that way as well as a shoulder problem. In terns of an infection, odd presentation for a lung or respiratory problem. You should follow up for another evaluation by your pcp.

See below: Neck pain that runs down the arm with or without numbness and tingling sounds like a pinched nerve in the neck. This is usually caused by a protruding disc or bone spur touching one of your spinal nerves as it leaves your neck. Your neck muscle are probably tensing up which can also cause headaches.
