Swllen lip: Not sure what causes the swelling to be severe as i gather from your question, and the split, i would highly recommend seek the help of a dentist asap. Canker sore cuses some swelling not severe, .

Stomatitis : He may have stomatitis where the tissues of the mouth swell and multiple canker sores can appear. He might also have bite trauma. We tend to 'play' with canker sores so get the soreness out. Please see your dentist or doctor so they can prescribe an antiviral to help your brother heal or go to the emergency room since his lip is so swollen.
