Repeat thyroid tests: For someone w/o prior thyroid disease or taking thyroid pills -- a new finding of high TSH suggests possible hypothyroidism. This may be accompanied by a low free T4 (overt hypothyroidism) or normal range free T4 (subclinical hypothyroidism). Acute illness including infections can make all thyroid labs including TSH and free T4 behave unexpectedly. In this case, should repeat tests after recovery.

Hypothyroid: The TSH is 10 times more sensitive as a test of hypothyroidism than the T4 because it is reported on what is called a 'log' scale. The high T4 can be cause by hormones such as birth control pills even if you are hypothyroid. A free T4 will be low or low-normal.
