A few things...: First, there could have been a false negative test for the fungus. It could also be due to trauma, either a direct hit or repeatative microtrauma (like constant rubbing in the shoe), or possibly psoriasis.

Dystrophic nail: First of all it still could have been caused by a fungus and the test could be a false negative. Nails can fall off from injury. There are numerous conditions that could cause this. I would want to know if the entire nail fell off or just part. You could try using nail polish with antifungal medicine added. These can be found on the web. It usually takes about 6 months for a new nail to grow.
