Depends: If you have a perforation with drainage, sometimes just local antibiotic drops will resolve the problem. If the drainage has a foul smell, you may have a cyst of skin called a cholesteatoma and that requires surgical removal. Csom is a generic term. Therefore, ask your otologist exactly what the problem is and why he/she thinks surgery is necessary.
