Several are possible: There could be several causes given the distribution of the discomfort. I have referred your question. I do think our doctors would like to know a little more. Can you tell use what you are doing when the pain occurs? How often have you experienced it? Also what have you tried to ease the pain? You may add more as a comment. Thanks for asking.

Rib or myofascial : Hisdelucas, it could be rib pain or myofascial (muscle and fascia) pain. The distribution you describe suggests something is wrong at thoracic vertebra 4. Base of the neck is something else. Either the two are from postural imbalance or something is going on at the spinal cord. See a pain specialist or your primary care doctor.
