Many possibilities: Has there been injury? Or any history of arthritis? Without these, the most likely explanation is tendonitis. I recommend an evaluation by a sports medicine specialist, for exam and x-rays. Currently, you could try motrin, ice/heat alternating, and night-time neutralization bracing. Wrap your arm with a pillow and sleep with it cushioned and semi-straight at your side.
