Not much: Mesh will be incorporated into body tissue , which will be main strength not mesh itself.

Mesh tear: Unlikely, more likely to move out of position or have a securing stitch brake or scar tissue causing pain or recurrence of hernia.

Almost zero.: The mesh itself almost never fails. But, what does happen is the mesh can separate from the tissue to which it is secured, and this would be considered a hernia repair failure. The failure rate of repair is dependent on many many factors. Inguinal hernia repair failure rates are very low, less than 2 percent. But ventral hernia failure rates can be anywhere from 2 to 50 percent.
