Epididymitis: The epididymis connects the testicle to the vas defrens which essentially carries the sperm out. With the epididymis infected and inflammed, it backs up the system and can cause the testicle to enlarge. Should get treated and can use hot soaks and anti-inflammatories to help.

Epididymo-orchitis.: You are correct in understanding that the epididymis is an appendage of the testicle. However, they are connected, and infections of the epididymis (epididymitis) can lead to infection or at least inflammation of the testicle (orchitis). This inflammatory response causes pain, fullness, swelling, and warmth in the scrotum. He needs evaluation to make sure he is getting appropriately treated.
