Works on the liver: Metformin is a biguanide and it works to reduce glucose synthesis by the liver during the fasting state. Type 2 diabetes is viewed by the liver as a state of cellular deprivation of glucose due to changes in receptors for Insulin action on these cells preventing entry of glucose in the cellular compartment, or Insulin resistance.
