MDT: Mean duration of gas exchange which increases the elimination of co2. Current strategies for ventilation in ARDS include low tidal volumes with rapid respiratory rates and permissive hypercapnia, which leads to a respiratory acidosis in most patients. Increasing the inspiratory pause or including one periodically decreases pac02 by increasing gas exchange time. Survival effects are not studied.

Plateau pressure: Intensivists can dial in an inspiratory pause on the ventilator to measure plateau pressure. Plateau pressure reflects pressure in the alveoli. Studies show that keeping the plateau pressure below a certain level can improve patient outcomes with ards.
