Doctor can evaluate: A doctor will have to get more of the story about the nipples, and do an exam (one can start with his primary care doctor). Nipple rashes can be from friction (from shirts, sports equipment like swimming in an inner tube, etc...), eczema, cancer, etc... Nipple discharges can be from hormonal imbalances, which may be due to hormone-producing glands, liver disease, medications, etc...
