Controversial: Some lung doctors or pulmonologists, don't think this is a good idea. See the article about antibiotic use in the management of pulmonary nodules in the journal chest from 2010.

Yes,poss infection?: Referral to a specialist means biopsy versus observation with follow up ct. It is okay to treat for poss infection and see if gets better before getting aggressive with it. Biopsy could have complications and could be not diagnostic. However this is not standard, some lesions may look like cancer from the first ct . Yours sounds like not straightforward.

Yes: It really depends on your medical history and risk factors for cancer. If your doctor saw something on the CT scan which based on your medical history looked as though it could be an infection, then it would be appropriate to try a course of antibiotics and repeat imaging sometime in the future before becoming worried and sending you to a specialist.
