Many reasons.: A nephrostomy tube allows urine to drain from the kidney out to a drainage bag. As it relies on gravity to drain, body position and tube orientation can affect drainage. If the tube is blocked, it will not drain. Finally, urine will take the path of least resistance. If the native ureter can drain the kidney, or if urine is leaking from the kidney (urinoma), the tube will not drain well.
