With many blackheads, a retinoic acid derivative is the best start; our correspondent's personal physician might add topical Clindamycin as a next step prior to a dermatology referral. Good old benzoyl peroxide is another possibility. If we don't have good results in 4 weeks, some primary care physicians are comfortable writing for a systemic antibiotic like tetracycline.
