In lupus, what is likely cause of sharp chest pain i get sometimes during flares that worsens when i lean forward or inhale. heart/lungs scans normal. ?

Unclear: Lupus pts can get pericarditis - sharp chest pain which is worse with deep breath but improved by leaning forward/worse lying on your back. Alternatively, lupus pts also get myalgia (muscle pain) and arthralgia (joint pain, including of the rib cage) which can be worse with deep breath but wouldn't change much with different position. Heart and lung scans would be normal for all of these.

Musculoskeletal pain: Most probably musculoskeletal pain. However this needs full evaluation with your doctor, though the heart and lung ' scans' normal.
