No: It should not. There are many reasons why people have problems while married. This reason, unless is as big as a basketball (and yes I have taken care of such a big hydrocele), i don't see why this should be a reason.

Could be psychologic: Wife may be "turned off" pscghologically by appearance of a significantly enlarged scrotum, as can the husband. Functionally a large hydrocele can affect distance the penis protrudes & interfere with insertion or with depth of penetration. Discuss with each other, marriage may require counseling for other reasons. You are young, why not see a urologist and get in taken care of?
