Yes: They would check but not all knots are seen. They usually don't tighten until delivery and rarely are problems.

May not be able to s: See knots on ultrasound, if u are worried your doc can send u to a perinatologist for an ultrasound but usually u cannot see knots in the cord.

Not normally: The norm is to confirm that the umbilical cord contains three vessels. Because the umbilical vessels are so tortuous and twisted even in a normal cord - it is not possible to identify which cord actually has a true knot. Many umbilical cords have segments that would appear to be knotted, but have only a false knot. There is no recommendation for managing the pregnancy any differently.
