Infection: Since you are having tenderness and irritation would be concerned about infection called balantitis. Infection of foreskin. Need to be seen and evaluated, of couse good hygiene is always good for the foreskin.

Smegma: Normal accretions under the foreskin. Be sure to retract your foreskin when you urinate and wash. If that isn't enough, your doctor can prescribe topical medication to clear up residual irritation.
