Needs attention: First you should stop smoking especially you know that it causes cancer. Any swollen lymph gland ( if it is is a ) concern, needs exam by your doctor if it i due to throat infection will treat you with antibiotics and local care other wise will refer you to ENT specialist for endoscopic examination.

Need to examine: It is not feasible to provide a meaningful opinion without additional history, physical examination and may be some tests. It would be prudent to see an ENT doctor.
