Nephrologist: If you have a chronic kidney disease and need a kidney transplantation, please have your PCP (primary care physician) to refer you to a nephrologist (a kidney specialist) who will guide you in the right direction.

UNOS: UNOS has information on all transplant centers in USA. http://www.unos.org. Look up centers in your area, compare their results and then discuss with your Kidney doctor and pick a center. Look at outcomes, patient and graft survival. best of luck.
