Angiogram: Sounds like a coronary angiogram or cardiac cta is in order to better sort this out.

Chest pain: It seems a long time for costochondritis but another thought is pericarditis which can persist after surgery. Most importantly, would rule out coronary blockage as a cause of the discomfort.
