What causes people to have high cholesterol?

Depends: For many it is genetic - you get it from your parents. Eating poorly can also cause a bad lipid panel: high amounts of sugar, saturated fat, processed foods and smoking can all have a negative impact on lipids. Some, however, eat and exercise perfectly and still have a horrid lipid panel - that's where medicines can help!
