Pain & redness: Increasing pain at the wound with swelling, skin becomes warm and develops spreading redness. May get bad looking drainage from the site. Sometimes person can get fevers.

Red, drainage, fever: If a wound has redness, warm to touch, purilent yellow/green drainage, fever, increasing pain, or just fails to heal as expected.

Visual: Primarly just looking at the wound, redness of the wound, draining fluid (or pus), pain, swelling, and loss of function of the body part, e.g. a finger that is too sore to move, increase in size or change in a chronic wound. Blood tests are sometimes ordered if the diagnosis is unclear.
