Hormones, or fibroid: Could be your hormones being messed up, you could have a fibroid or polyp or have too mcuh endometrial tissue that is breaking down that could be associated with pre- cancer or uncommonly cancer of the endometrium, so see your gyn for a workup.

Depends on: : For a woman who is forty, some more common causes are hormonal changes our bodies experience with aging, these can predispose to polyps (or to precancerous conditions ) - so you should see a dr, have a sono and biopsy if indicated by sono. Could be fibroid tumors (benign muscle balls growing in uterus that can disrupt normal bleeding), or ovarian cysts, or thyroid problems. See ur gynecologist.

Need to be seen: With this kind of bleeding for this long you need to a gyn. There are number of things it could be, among them hormonal imbalances, fibroids, pregnancy. Your doctor can help sort this out and rule out anything serious.
