Referred pain: Assuming you've had your heart checked out and all is well, it may indeed be gerd (reflux, or heartburn) or an ulcer. Coffee is known for decreasing the tension of the sphincter separating the esophagus and stomach, allowing liquid to flash back up into the esophagus. Nerves that supply the esophagus (as well as heart, gall bladder, and diaphragm) can refer pain to the arm (or shoulder or jaw).
