See this NIH webpage: Tarlov cysts are sacs filled with fluid that most often affect nerve roots in the sacrum, the bones at the base of the spine. These cysts (also known as meningeal or perineural cysts) can compress nerve roots, causing lower back pain, sciatica (pain in the lower back, buttocks, and down a leg), urine incontinence, headaches, etc... http://www.ninds.nih.gov/disorders/tarlov_cysts/tarlov_c

Observation: Perineural, or Tarlov cysts are usually incidental findings on an MRI. They are usually asymptomatic and appear on an MRI. They are essentially thinned out parts of the thecal sac which contains cerebral spinal fluid. If you are not having any neurologic deficits, I would not intervene surgically. The covering, or dura is very thin, and would likely tear should one try to repair a cyst.
