Nothing: Most opioids or narcotics are metabolized by the liver and excreted by the kidneys as inactive metabolites. What this means is that the active part of the narcotic is transformed when it gets to the liver to an inactive form and then goes back into the blood stream and you pee it out. So its the health or function of you liver that is is important. A cirrhotic liver is less functional.
