Yes: If a pacemaker lead perforated the right atrial or ventricular wall....If suspected echocardiogram should be ordered immediately.

Yes: If the wire puntures the heart, blood fills the sac surrounding the heart and tamponade can result. This is a known risk of the procedure and occurs ~1:500 times.
