See a specialist: As you've tried for 18 months, i recommend seeing a reproductive endocrinology and infertility specialist. We used to say that 15-20% of couples had 'unexplained infertility' with regular cycles and normal tests. With modern fertility testing and checking for low egg supply, it's rare that we find zero issues. Rei docs offer more treatment options, and will return your wife to her OB when pregnant.

Depends: Ideally you would like to see a fertility specialist, especially after trying for over 1 year. Your OB should technically refer you to one at this point.
